Base for Primary Care Mental Health Service, administration staff and Diabetes Nurse Specialist.
The Primary Care Mental Health Team provide a GP Practice based service to those with mild to moderate mental health problems.
The Diabetes Nurse Specialist provies home/base service for those patients with diabetes who require inititation of insulin and oral medication/monitoring/advice/education and all other aspects of disease management.
